Sam's recurring nightmare of his last convo with Dean was one of the first images that came to me. And because I had so few words to work with, I had to really choose which lines to include, so I choose the ones that what Dean said would hurt Sam the most (I am a Sam!whump whore, the more he's hurt and tortured the more I love him) and definitely him saying Goodbye and hanging up must have torn Sam's heart out and was a major motivation to battle Lucifer.
