Wow, everything about this is just stunningly gorgeous. The blood splatters on the wall and on Sam's fingers and arm, Sam's delectable grip on Dean's throat, Dean's facial expression (and not to mention his freckles!), the bloodstained knife lying in the corner. WOW. This is sooo unbelievably hot.
